Because any action performed with your offhand suffers a -20 penalty. The attack rule quoted is just a specific example, not an exclusive example. Trying to paint with your offhand? -20. Throw a dagger? -20. Parry an attack? -20.

Also, something to keep in mind is that Stevedores have WS as a career attribute. I think the problem with that house rule is that IIRC there are talents that basically give you that bonus as the primary feature of the talent. Yes, you can use Melee (Brawling) untrained, so Dirty Fighting would help with that. You’d have a character who knows a few sneaky moves, but isn’t really a trained brawler. Something to consider regarding your first point is that in 4ed there’s no requirement for a priest to have the Miracle talents. You only need 1 career talent to finish the career, so an NPC can have one of the others.
