As a personal thing, I dislike mass transit in RPGs that is cheap & fast & safe. I feel it takes a lot of the adventure out of exploration and removes a lot of potential for adventure hooks. To that end, I have removed all Realmgates that go from one realm to somewhere else within that realm...

All good questions, and I don't really know either. But if I had to make something up, I would say that when mortals die and go to Sigmar for judgement (let's say, substitute Alarielle, Grungni, Teclis, etc as needed), he picks some out and then imbues them with a spark of his essence 'binding' them...

It's been a while, so someone please correct me if necessesary, but to "flip" the dice is to reverse the numbers. "Flip to fail" would be to take the 38 that you rolled and treat it as a 83 if that would make your fail. "Flip to succeed" is the opposite.

To assess a wound, dig a bandage out of a pouch/pocket, apply the bandage, and get the recipient ready to take other actions? Would take at least 30 seconds to a minute. I would add in a third category: Emotionally Mature. Here's how I would break it down: Physically Mature: Fullness of bodily growth and sexually capable of reproduction and generally surviving gestating a baby to term (for females), in humans it's about 15-18. For elves, I would say it's about 18-20...
